He was diagnosed with schizophrenia/depression at age 11, getting hospitalized at least once after a breakdown at school because of it. He went through countless therapists and medicines since, but they all just never worked out, and it took a dark toll on his sanity.
While passed out drunk at age 19, he later woke up to find his two younger sisters, Hannah and Beth (whom he clearly loved dearly) ran outside into a snowstorm after a prank from his "friends" gone wrong, never to be seen again; the prank amounted to catfishing one sister into taking her shirt off on camera. He even tried to kill himself because of this, and it's also more or less implied that his parents are neglectful towards him.
In the end, he's the only one of the eight main protagonists who can't be saved regardless of player choice, either getting his head crushed by his own sister who turned into a Wendigo, or turning into a Wendigo himself. The latter fate is heavily implied to be worse than death, your mind being submerged within your own body as the cannibal demon possessing it slaughters people against your will. I wouldn't wish either ending upon even my worst enemy.
Excluding these points, he's one of the only two main protagonists in the entire story (alongside Jessica) who never even thinks about murdering someone, so he has no real MEH crossings. It's really saying something when even Sam and Chris, the two people Josh hurt the most (and both of whom had nothing to do with the prank that killed his sisters) still sympathize and wanna help him after everything he did.

Honestly, Infinite the Jackal.
Tai Lung had the saddest. He was abandoned as a child and raised by Shifu. But Shifu inflicted harsh training on him. He still kept on trying to impress him and all of these efforts were nothing when Oogway doesn't select him to be the Dragon Warrior.
Runner up is Darth Maul, who was taken away from his mother and groomed into a Sith Lord. Then his legs were cut off and he eventually becomes a crime lord in an attempt to go back to Sidious's rule. However, they were all for nothing Sidious abandoned him, killed his family members and later tortured him to death. He tries getting Ahsoka to join him in ending Palpatine's rule but she refuses. Later, Maul tries get Ezra to join and uses extreme methods to get him to his side, but in the end, he also refuses. Overall, he was forced into the world of darkness with no way out of it and it's best shown when he dies that he's a victim.

Jason Todd was tortured to death by the Joker, and when he came back from the dead, he had to live his life knowing that his mentor doesn't feel like killing his killer.
Arthur Fleck was born to an alcoholic mother who let his father abuse him, in addition to having a condition that forced him to laugh at the wrong time. As an adult, he was abused and shunned by his environment with the exception of a few good people. Unlike most Jokers, Fleck didn't do anything to deserve it.
The novel version of Dennis Nedry was working for the novel version of John Hammond, who was a very bad boss, and sufferes both financially and mentally. He was also exploited by Dodgson, who was more than likely planning to kill him after getting the goods. He wasn't nearly as evil as the cinematic Nedry. He wanted to bring the goods and get back and fix everything. Except the dilophosaurus caught him first. It was even less pretty that in the movie.
The Nolanverse version of Two-Face used to be a genuinely good lawyer named Harvey Dent, who wanted the best for Gotham and was Joker's second arch-enemy, until Joker killed his girlfriend and burned half of his face and brain during a sadistic choice game. I admit trying to kill James Gordon's child was a bit messed-up, but other than that, Two-Face had some solid points. If you don't count Catwoman as a villain, than Two-Face is the only sympathetic Nolanverse villain.
The hunchback of "300 Spartans" (forgot how to spell his name) was borned deformed in Sparta and was shunned by the harsh society for most of his life. Xerses took a ride on his misery, and living his life knowing his betrayal left him in a fate worse than death.
Tyrion Lannister was born as a dwarf to Tywim Lannister and a mother who died shortly after his birth. Twyin basically hated him because he was born, blaming him at the death of his own mother. Growing up at Tywin couldn't be an enjoyable experience either way. Let's just say there's a good reason for why Tyrion is the most sympathetic "Game Of Thrones" villain.
