Riddler from The Batman Saga. Even though his entire plan wasn't successful, he still managed to kill several figures in Gotham, as well as cause mass destruction by flooding Gotham city even after getting arrested.
How was Jack successful ? He did not get his wish granted.
Anyway, Death and Riddler were mostly successful in their plans. Yeah, he didn't kill Puss but he took away Jack Horner, who took the lives of his fellow people granted.
