Marvel Comics heinous standard is so insanely high that it's impossible to name all the most evil marvel comics villains, but I'd say the Inheritors are up there. They are a group of hunters that attempted to kill all the Spider-Men in the multiverse.
One of them, Bora, did the unforgivable: killing Spider-Cat (said cat was later revived, but the action is still unforgivable)
In terms of mentality, Roger from Lord of the Flies is more evil than Jack. Though Jack committed more crimes, Roger is Approved Pure Evil, and is way more sadistic.
Main characters with external conflicts is good, but Horror Survival is probably best.
As I just said, I'm not a fan of Horror based on Real Events, so my answer would be I definitely like Horror Fiction with protagonists and antagonists more.
Not a huge fan of Horror based on real events. As I usually enjoy being a fan of villains, you can't truly ever like a Horror villain that's based on a real life bad person. I prefer fictional horror.
Eldritch and superorganisms are perfect horror antagonists, as well as just normal humans in certain scenarios.

Much like with the rest of the sequel trilogy, his role in The Force Awakens was good, but kind of went down hill after that.
He's an extremely interesting character, though I think most people that know about him haven't actually taken the time to read Blood Meridian. I highly recommend that people do, though, because it's a really interesting novel.

The only villain I can think of is Dr. James Sheridan. He was once considered a possible Pure Good, but then became Pure Evil.
However if you want some examples that aren't 'approved pure good to approved pure evil', I have some more examples, though these aren't quite as accurate to the original question as Sheridan is.
Absolute Solver (Tessa Elliot is a fairly good character, but it is revealed that the non-flashback Tessa Elliot turns about to be the Absolute Solver wearing Tessa's skin as a disguise, and the Solver is Pure Evil.)
Lex Luthor (Has both Pure Good and Pure Evil counterparts)
Lloyd Bailey (While under the alias of the Masked Man, Lloyd pretended to be his twin brother, John Bailey, who is extremely good. Lloyd himself is Pure Evil.)
