Death is a boss that can appear in all environments of Chapter 4 (Womb, Utero, Scarred Womb).
Appearance
Death is an eyeless, gray skeleton-like being with a skinny, pale grey body and a hollow skull, riding a skeletal hobby horse. His blood is also completely black. He will surround himself with the Grim Reaper's trademark weapon, the scythe.
Behavior
- pawns four Death Scythes along the walls of the room, aligned with Isaac in the cardinal directions. After a few seconds, they will accelerate towards Isaac.
- If Isaac is close to a wall, the scythe that would normally be spawned on that wall does not spawn.
- Spawns two Death Scythes in random positions around the room that constantly home in on Isaac until they are destroyed.
- Uses The Hourglass to slow Isaac down.
Phase 2[]
After enough damage is dealt, Death will dismount from his horse. The horse has 150 HP, but he will also lose health when Death is damaged. Attacking the horse does not damage Death.
- Death occasionally spawns two Knights and can have a maximum of two.
- Death's horse continuously flies horizontally across the room going off-screen and continuing on from the opposite end of the screen.
- His charging patterns occur along 3 paths, starting with an upper charge into a middle charge a short distance below and finally a lower charge an identical distance below the middle path. He then restarts this process and can perform his upper charge in a new position in the room.
- His first charge will always leave room below him to allow for the lower second and third charges.
- The horse continues to charge even after Death has been defeated.
- He deals heavy damage to any enemy he runs into, except Death.
- His charging patterns occur along 3 paths, starting with an upper charge into a middle charge a short distance below and finally a lower charge an identical distance below the middle path. He then restarts this process and can perform his upper charge in a new position in the room.
Both Death and his horse deal a whole heart of contact damage in both phases.
