The Nurses are recurring antagonists of the Silent Hill franchise.
They are manifestations of psychological trauma, especially guilt, lust, repression, and illness. The town of Silent Hill has the supernatural ability to turn inner fears and subconscious thoughts into physical reality. They common represent sexual repression or guilt, medical trauma or fear of hospitals, and guilt surrounding caregiving or neglect.

Nurse Lisa Garland
The "Puppet Nurses" from the Alchemilla Hospital are linked to Lisa Garland, a nurse who worked at Alchemilla and cared for Alessa Gillespie. Garland was originally manipulated by Dr. Michael Kaufmann and kept compliant through drugs. They represent drug use, loss of control, and corrupted innocence. These "Puppet Nurses" are also possessed victims of a parasitic infestation, similar to the "Puppet Doctors."
Silent Hill 2 (2001)[]
The "Bubble Head Nurses" from the Brookhaven Hospital represent James Sunderland's sexual frustration and guilt over his terminally ill wife Mary, as well as his distorted sexual desire mixed with guilt, as James likely saw nurses as both caregivers and forbidden objects of lust. They wear skimpy outfits, had twitching movements, are agressive, and often wield blunt weapons like steel pipes. Their facelessness (bubble-like heads) may reflect dehumanization and emotional detachment. They are designed by Masahiro Ito.
Silent Hill 3 (2003)[]
The "Slurper Nurses" from the Brookhaven and Otherworld hospitals are linked to Heather Mason (Alessa's reincarnation). They represent the lingering trauma of Alessa's time in the hospital, pain, suffering, and isolation.
Silent Hill: Origins (2007)[]
The Nurses return again in skimpy uniforms and aggressive behavior. They are linked to Travis Grady and represent his mental health struggles and mother's institutionalization.
Silent Hill: Homecoming (2008)[]
The Nurses reflect Alex Shepherd's time in hospitals and his repressed trauma, especially guilt surrounding his brother. They also represent his conflicted feelings toward women, including sexual confusion and fear. They attack when the player shine its flashlight or make noise, tying into the film's depiction.
Silent Hill Movies[]
The Silent Hill movie and its sequel feature "Dark Nurses", which are blind and hyper-sensitive to sound. Their exaggerated sexualized designs were heavily inspired by the Bubble Head Nurses from SH2. In the film, they represent the twisting of feminine caregivers into something monstrous, fitting the religious cult's perversion of morality and purity. The film's portrayal also heavily influenced future games, like Homecoming and Origins.
