Pinhead is the main antagonist of the 2018 horror film Hellraiser: Judgment which is unrelated to the previous entries in the Hellraiser film series. in this film, Pinhead is a representative of the cenobites in the Labyrinth faction of hell and after a detective and serial killer named Sean Carter escapes from hell by using The Lament Configuration, hence being targeted by a faction from hell, summons Pinhead to attempt to make a deal with him to provide him victims in hopes of saving his soul.
He was portrayed by Paul T. Taylor in his first villainous role.
Biography[]
In hell, Pinhead represents the Cenobites and the Labyrinth faction of hell and during a council meeting with the Stygian Inquisition are discussing various ways of harvesting souls in hell while technology is advancing among the human race on Earth. Meanwhile, a detective and serial killer Sean Carter is sent to hell for his sins but his damnation is opposed by the angel Jophiel. He manages to escape hell through the use of the Lament Configuration. The Auditor is tasked to find him and bring him back to hell, as a result, he seeks guidance from Pinhead on tracking him down. On earth, Sean discovers that his wife Allison is having an affair with his brother David and holds them hostage.
Sean opens the box and summons Pinhead, the cenobites and The Auditor and offers David and Allison to Pinhead in exchange for his soul. Pinhead tells him they will be dealt with for opening the box but, because a separate faction of hell wanted his soul, no deal will be made. The Auditor tells Sean that he has been found guilty of his sins and will be promptly sent back to hell. Jophiel intervenes again and reveals to Pinhead and the Auditor that Sean is part of God's plan to instill fear into sinners on Earth. Pinhead arranges for Sean's previous victim Christine to kill Sean. Jophiel threatens Pinhead for his defiance which angers him, he would then proceed to use his hook chains to tear her apart. As punishment, God expels Pinhead from hell and forces him to walk the earth as a mortal man as he screams in agonizing defeat.

- Doug Bradley was offered to return to the role of Pinhead for Hellraiser: Judgment but turned it down due to its script and low budget. Hence, removing any relation to the previous Hellraiser films and is treated as a separate entity much like Hellraiser: Revelations.
- If the sequel to Hellraiser: Judgement been greenlit, it would've focused on Pinhead as a mortal man becoming a serial killer as well as tracking the Lament Configuration in hopes of being sent back to hell and be turned into a cenobite once again.
