In Bio Inc trailer,the player appear as a doctor visiting a patient named John.They first telling him about the bad news: his high cholesterol and high blood pressure lead to him having chest pain.Addition of being a smoker and overweight make John lead to him having stroke.And his insomnia make him more vulnerable to bacteria and virus.At the end,John ask the doctor about will he make him feeling better,which the doctor response with "Who say I was here to help you?" thus revealing his true nature.
In Bio Inc Redemption trailer,the player have resurrected John back,but only to test on him once again by giving him various diseases and risk factors and kill him again.

The player will have to kill their victim by collecting Bio Points and evolving Diseases and Risk Factors.If a diseases is evolved,it will start damaging the victim's systems and if a Risk Factors is evolved,it will boost the damage.If a victim's system collapses,it will continously damage the healthy system.And when all system are terminated,the player wins the stage.
The player can also make the victim suicide by evolving Unlucky,Dementia and Major Depression.If done correctly,a ingame popup will state the victim is feeling depressed and having suicidal thought.If RNG is on the player's side,the victim will suicide,ending their life and the player wins.
The player has a major enemy to worry about:the Recovery.When the victim's symptom become bad or if they have an annual routine checkup,they will visit to the doctor.Once they visit to the doctor,the recovery progress start increase.When the victim is in critical condition,they will visit the emergency room,which will boost the recovery progress.The victim can also get medication to prevent their system from collapsing.If the recovery reaches 100%,the player loses and all systems regenerate at the speed of 15% per day and all diseases evolved are rendered null.They also lose when fails to complete the stage when run out of time during specific stage.
The player can combat the Recovery by evolving items in the Recovery Factors.Nosocomephobia I & II prevents the victim from visiting to the doctors temporarily,Countryside prevents the victim from visiting the emergency room temporarily,Bad Doctors I & II slow down the recovery progress,Bad Diagnosis and Surgical decrease it and Nurses on Strike & Doctors on Strike stop it temporarily.And Placebos made medication useless,while Placebos (Poison) make it counter-productive.
Bio Inc Redemption.[]
In this game,everything work relatively the same, but the doctor in this version will diagnosis and treat diseases,rendered them null and when the treatment completed,the recovery increased depend on the diseases that are treated.Unlike the first game, terminate all systems is no longer mandatory to kill the victim (with the exception of Total Failure, Road Block and Death Race II). Also,if the player fails,the victim will just sigh of relief. Evolving Risk Factors on this game now also made all diseases in a system's cost decrease.
While the Recovery Factors stays mostly the same,the Placebos & Placebos (Poison) has been removed.They are replace with Generic Medication,which made all treatments 10% less effective,and Treatment Pause,which stop all already prescribed treatments for a week.

- It's possible that the players suffered from Multi-personality disorder, due to having two sides of one being a noble doctor and another is a psychopathic scientist, and both are portrayed as very knowledge in term of medical.
