The Player is the main protagonist of Plague Inc. and its sequel, Plague Inc Evolved.

The player’s goal would be to kill all people inside the world using a deadly disease. There are currently 11 diseases available: Bacteria, Virus, Fungus, Parasite, Prion, Nano-Virus, Bio-Weapon, Neurax Worm, Necroa Virus, Simian Flu and Shadow Plague, with the last 4 being classified as special diseases.
To start the game, they must select a country to infect a Patient Zero. To enhance the disease, they must invest a currency called DNA Points into 3 trees: Transmissions, Symptoms and Abilities. Transmissions focus on increase Infectivity, Symptoms increase Severity and Lethality, and Abilities enhance the disease against climates, medicines and fight the cure.
There are 3 core stats: Infectivity, Severity and Lethality. Infectivity is how quickly the diseases spread, Severity increase your DNA Points income and slow down the cure at the downside of being easier to detect, and Lethality is how quickly the diseases kills.
As the diseases spread inside their starting country, they will begin to infect other countries through ships, planes and land borders.
At any point when the player infect a high number of people, they will be detected by humans. This is when their biggest enemy is in play: The Cure. The more the player managed to infect, the more the world will invest money into the cure. The player can combat the cure inside the Abilities tab, which include Genetic Hardening, which slow down the cure, and Genetic Reshuffle, which reset the cure by increase the requirement. Some diseases like Prion and Nano-Virus possess extra anti-cure measures.
The player will win if (usually) everyone are dead, but it will be game over if the cure reach 100% or if all infected dies before the whole world is infected.
In the Neurax Worm, by using Transcendence, the player can win by having everyone in the world worshipping the Neurax Worm, thus the killing requirement is removed, although killing everyone is still a possible choice.
In the Necroa Virus, the disease itself is very easy to cure, however zombies are invulnerable to the cure and can continue to spread, thus allowing the game to continue even with the cure at 100%, although the player would have to deal with an organization named Z-Com from that point onward, with not being able to defeat them end the game.
In the Simian Flu, the player can infect both humans and apes. The goals is to having the human population outnumbered by apes, thus making apes the dominant species. A new enemy called Gen-Sys is introduced that research on apes to boost the cure, and can destroy ape colonies. When they build a lab in any countries, it is possible for the player to destroy it with apes, which also reset the cure.
In the Shadow Plague, the Abilities tab is removed, in its place is the Vampire tab, which revolve about boosting your vampire. Unlike other diseases, the game does not start by infecting a human, but instead creating a vampire. There are no Transmissions, and only one free Symptom called Shadow Blessing, which allow the vampire to spread the Shadow Plague. Once it is evolved, some Symptoms, Vampire Abilities and many Transmissions are also unlocked. Vampires can build lairs, killing people for DNA, or flying around the world. They also combat the Templars, the enemies of this diseases, by destroying their fort that pop out once the vampire is detected. They can also destroy WHO lab that randomly pop out once Shadow Blessing is evolved. And finally, the player can make more vampires by having a large number of infected killed to find a host. The goal is either evolve Shadow Slaves so that every infected worship the vampires, or kill them all.

- The player of Plague Inc. is one of the most iconic example of a villainous protagonist.
- As for now, it remain unknown as to why the player did what they did, nor do it is known who they are, although it is possible that the player is evil just because it’s fun.
- Judging by their vast knowledge over biology, the player is likely a scientist or a biologist, and they may live in another planet like Moon (which explain why they knew so much about what happens in the Earth, as well as to why the player is immune to their disease).
- Santa’s Little Helper is currently the only stage outside of Cure Mode where the player is a heroic figure, due to opposing the evil government.
