The Reaper Nurses are a group of minor antagonists in Dark Deception, making their first appearance in Chapter 4 as the secondary antagonists of "Torment Therapy"'. They share the level with The Matron.
The Reaper Nurses appear as slim, female humanoids with light skin dressed in 20th-century nurse clothing. Their faces are visibly obscured by what appears to be a white paper bag with a medical cross marked on the front, though they are still able to see. Like the Matron, the Reaper Nurses wear red combs across the top of their bags. While it is unknown what their faces truly look like, some merchandise of them has shown that they actually wear pink lipstick and appear to have the mouth and chin of a normal human.
Their clothing comprises of a red and white dress, with red buttons on the shirt and the skirt, and white leggings on their legs, with black first-aid signs featured on the shoulders and belt. They wear red gloves, and red roller skates resembling heels. Their most prominent feature is the oversized syringe they wield, which they use to stab their victims. The reaper nurses will have three weapons: a giant syringe that serves as their main weapon, a smaller syringe, and a pill. Their kill animation only uses a giant syringe nonetheless. Once the Nurses go into Nightmare Mode, their clothing turns red and black. They are fast enough to catch up to speeding ambulances.
They are voiced by Tara Strong, who also voices Harley Quinn in various media and Iggy "Hop" Koopa in The Adventures of Super Mario Bros 3 and Super Mario World.

During the main level, Reaper Nurses will wander the halls while cloaked, their position are given by skating sounds and red flashes going off with every step; they will also occasionally spout dialogue. If the player gets close enough to a Reaper Nurse, she will begin to chase them at a rate faster than the player's run speed, decloaking themselves once they get close enough and killing the player on contact. If the player is out of the reach of their syringe, the Reaper Nurse will throw a pill at the player instead, which stuns them on contact; the player needs to mash the F key to recover. The Reaper Nurses are also attracted to sound, moving to the player's location should they hear them opening a double door.
The overall AI of the reaper nurses are somewhat a mixture of Agatha (In that they travel fast when aware of the player's location, slow when not, and they are well-focused on any sounds the player makes) and the Clown Gremlins (in that they are on lookout for the player in large numbers, and can communicate with each other).
During the Matron's boss battle, the Reaper Nurses in their Nightmare Mode forms will skate down the road, keeping pace with the ambulances the player fights on. Occasionally, they will jump onto an ambulance in an attempt to attack the player. The player must first stun them before hitting them off the ambulance; doing so while the Reaper Nurse is conscious will have her block the attack.

The Reaper Nurses demonstrate a friendly, cheerful, and seductive personality, greeting the player as a new "patient". The Reaper Nurses have an extremely deceptive and sadistic side, however, saying they get carried away sometimes in performing their duties. One of them tries to mislead the player into various life-ending traps.
Reaper Nurses also hate men, demonstrated by some of their voicelines and the graffitis painted on the prison area (presumably by them). The main character, Doug Houser, is male in nature, so this fact makes sense. Although they likely don’t entirely hate men and likely have slept with a lot of them over the years to satisfy their pleasure. This is evident due to how they dance and behave.
Females, however, are treated with genuine hospitality and kindness. If a female "checks all the boxes" in their eyes, the Reaper Nurses will offer to make the female a Reaper Nurse as well, remaining amicable even if the offer is declined. However, the Reaper Nurses will not hesitate to become hostile towards a fellow female if she impedes their duties or trespasses into the restricted areas of the hospital.
