The Doctor (Russian: Доктор or Врач) is one of the main antagonists (alongside Silantius and the Baron) of Metro Exodus. He is the leader of the Cannibals in the Yamantau bunker.
He was voiced by Mark Ivanir, who also portrayed Mikael Abramov in Law & Order: Organized Crime and Solomon in Battlefield 3.

Past[]
The Doctor originally worked as a doctor in the Yamantau bunker during its construction, as his moniker suggests. When the explosives detonated, he and about a thousand other survivors were trapped within the bunker. A few communications officers and the most of them were employees. After a while, the party ran out of food and had to turn to cannibalism in order to survive.
Using the freshly fixed antennae to entice further survivors to the bunker so the cannibals could eat them was probably the Doctor's idea. In any event, by 2035, the Doctor—one of the few people in the bunker to have kept his higher brain functions and any sapience—emerged as the group's leader.
Metro Exodus[]
The Doctor initially appears in a recurring radio message asking survivors to assemble at the Yamantau bunker as part of the "Ark restoration project" shortly after the Aurora and its crew escape from Moscow. After the Aurora crosses the Volga, Miller calls the Doctor directly on the radio, impersonating a government official (Deputy director of communications Major Ivanov).
Miller, Anna, and Artyom are taken to a room when the team arrives, where a glass wall divides them from the "ministers." They get quickly overrun by scores of cannibals and are knocked unconscious, realizing too late that something is amiss. When they awaken, the Doctor welcomes them and tells them the evil secret of the bunker.
He threatens Artyom and Miller with calling the rest of their team if they do not want Anna tormented as he takes her away to be tested for infections (owing to her cough). He departs just before the Rangers save Miller and Artyom. The Doctor knocks Artyom down with a metal rebar after he frees Anna, only to discover himself being held at knifepoint by Anna. He cries out for help but to no effect; a little while later, Anna slashes his throat.
When the Doctor checked Anna, he found that her lungs were disintegrating (as a result of the toxic gas she absorbed at the Volga weapons bunker), but Anna kept it a secret so as not to concern anybody until the symptoms had manifested fully later in the game.
