At the beginning of the episode, Karen obtains a virus due to her hanging out with a sick computer named Hanna. Plankton sees this and needs someone that is stupid enough to want to fix her, and just at the right time, SpongeBob is next to the Chum Bucket. After Plankton decides to call the misson "Operation: Save Karen", he transports SpongeBob into Karen so that he can defeat the Virus
Plankton gives SpongeBob directions on how to find the Virus and after he starts looking, he falls down a hole that was made by the Virus and sees it eat through Karen. Karen becomes completely delusional and goes to the Krusty Krab, where she starts causing chaos. SpongeBob flings back up to the surface of the inside of Karen and continues searching for the destruction caused by the Virus, however, he accidentally hits the security button. Thankfully, before SpongeBob gets hit by a laser, he gets saved by Ideal Plankton, who is what Karen pictures Plankton to be.
As the Virus continues destroying parts of Karen, SpongeBob and Ideal Plankton confront Off Model SpongeBob, Gnarly Squid and Money Krabs, and get into a battle with them. After SpongeBob and Ideal Plankton defeat them, they see the Virus on the other side of a trench. SpongeBob uses a rope to cross the trench, however, when Ideal Plankton tries to cross, part of the ground breaks and he falls into the trench, forcing SpongeBob to save Karen by himself.
Karen drives a car to a party at the Wagon Wheel because of how delusional she is. The Virus is shown eating Karen's memories, which prompts SpongeBob to defeat it. After his attempts fail, he gets so scared that he breaks into pieces. After going back to his normal form, he winds up with one piece left and throws on the ground. The Virus eats the piece, telling SpongeBob that the Virus is just hungry. The episode ends with SpongeBob taking out the Virus and letting him eat a Krabby Patty to satisfy his hunger before Plankton abandons it after SpongeBob confronts the Virus. Karen is still delusional but states that she likes it.
